ACO Population Health Management
Care management teams wanted to share a high level view of how they were performing and what metrics they would be measured on. We pulled the data from National Patient Care Opportunity Report to find the measurement metrics, used the financial report to compute and create targets and monthly health system summary report to pull information for actuals. For measures that have goals defined, we showed the goals on the card, so users can see where their organization stands with respect to regional goals.
